Award is in dark wood and is comprised of two major parts.

The curved rectangular base has an affixed gold engraved plate that reads:
Govenor's award to Nelson Mandela in appreciation for Anti-Apartheid and World Peace

The base holds a larger wooden plaque, by two silver poles, that includes two gold wreaths, a gold and blue Lions International logo, a yellow, gold, and red emblem that includes a small Lions International Logo with the words "we serve". Emblem's red border contains the words, "Towards an egalitarian society Dist. 323-A11990-91." Above the emblem, at the top of the plaque, is a gold star.
