White-coloured certificate mounted in a green folder with the seal of the House of Commons (Chambre des Communs) Canada embossed in gold on the front. Taken out of folder for preservation

Certificate has a maple leaf border and includes provincial seals from all Canadian provinces. The multi-colored House of Commons seal is printed at the top of the certificate and the gold seal of Jean Augustine, M.P. is located in the bottom-left corner.

The certificate reads:
